The Ultimate Secret Cow Level Guide - How to Unlock Everything

After years of "there is no cow level," there is, in fact, a cow level. The community (shoutout to the Not Finding A Cow Level Discord and xGarbett for the first confirmed entry) cracked the full quest chain within days of Lord of Hatred's launch, and it's an absolute love letter to the franchise's history.

This guide covers every step - from the three key items you need, to the Horadric Cube recipe, to actually entering the Cow Level, and how to bring your friends along for the ride.

Note: This quest chain spans content from the base game, Vessel of Hatred, and Lord of Hatred. Some steps involve very specific community-discovered sequences. Details are based on community research and may be refined as more is discovered.

What You Need - The Three Key Items

To unlock the Secret Cow Level, you need three items combined in the Horadric Cube:

1. Stamina Potion - from the base game cow quest chain
2. Rusted Bardiche - from the Vessel of Hatred cow quest chain
3. Neyrelle's Hand - from the Lord of Hatred cow quest chain

Both the Stamina Potion and Rusted Bardiche are tradeable and can be stashed in the Eternal Realm. If you farmed these during previous seasons, check your stash - you may already have them. If a friend has extras, they can trade them to you and you can skip straight to the Neyrelle's Hand step.

Step 1: The Stamina Potion (Base Game)

This is the original cow quest chain that's been in the game since launch. It revolves around collecting three relics that are references to Diablo 2's Secret Cow Level: Wirt's Leg (Bloody Wooden Shard), a piece of the Horadric Cube (Intricate Metallic Fragment), and a Tome of Town Portal (Musty Tome).

The Cow Counter

There is a hidden per-character kill counter that tracks cow kills across all regions. When your counter reaches 666 cow kills, you're guaranteed a relic drop. The region where you kill the 666th cow determines which relic you get:

• Intricate Metallic Fragment - 666th cow dies in the Dry Steppes
• Bloody Wooden Shard - 666th cow dies in Kehjistan or Hawezar
• Musty Tome - 666th cow dies in the Fractured Peaks or Scosglen

The counter is global, meaning any cow counts regardless of region. Cows killed by other players or enemies within a small range of you also count. This means you can farm the bulk of your kills in a convenient spot, then move to the region whose relic you need for the final kills.

After the guaranteed drop, additional relics can still drop on the same character, but at a very low RNG rate (estimated around 1% or less). It's usually faster to roll an alt for each guaranteed drop rather than grinding thousands of extra cows.

Important: Relics drop as global items on the ground. Other players nearby can pick them up if you're not careful.

Farming Routes

The most efficient cow farming methods involve quick loops through farm areas followed by a logout reset. Some popular routes:

• Farobru Route (Dry Steppes) - Exit south, loop through four farms around town for up to 14 cows per run. Resets in under 30 seconds.

• Cerrigar Route (Scosglen) - Loop southeast and southwest farms for up to 18 cows per run. Takes 1-2 minutes.

• Zarbinzet Route (Hawezar) - Southeast exit loop through three farms for up to 9 cows per run.

Pro tip: The "shmiga Reset Method" speeds things up significantly. Clear your route, log out (must be out of combat for instant logout), swap World Tier, log back in, and run the route in reverse. Each logout now alternates between the two World Tiers, respawning cows faster.

Cleansing the Relics

Once you have all three relics, take them to the fountain with four oxen statues in Ked Bardu. Drop the relics into the water to cleanse them, then pick them back up. The three previously inert Oxen God statues (North, West, South) will now become interactable. Interact with each one to offer a relic - you'll see a burst of white energy above each statue as you do.

After all three are offered, the Strange Key drops.

The Forlorn Hovel

The Strange Key grants access to the Forlorn Hovel, a hidden cellar on the very eastern tip of Scosglen (the part of the map that looks like a cow's head). Head there and use the key to open it. The key is consumed in the process.

Warning: If you lose the Strange Key before opening the Hovel, you'll need to start the entire relic process over.

Inside, you'll find cows that seem to be ignoring you, surrounded by dead villagers. Kill all the cows. The final one drops the Stamina Potion.

The Forlorn Hovel stays permanently unlocked after first entry. You can reset dungeons to farm additional Stamina Potions if needed. The potion is a Consumable (not a Quest Item), which means it's fully tradeable. Using it grants a 5% Movement Speed buff for approximately 5 minutes.

Step 2: The Rusted Bardiche (Vessel of Hatred)

This chain takes place entirely in Nahantu and involves three new relics - all references to Diablo 3's Staff of Herding crafting materials. Each relic has a hidden lore item that serves as a hint, tied to the story of "Lord Uacax."

Rusted Old Bell (Wirt's Bell)

Hint: "The Accession of Lord Uacax" found in Samuk

The lore reads: "On the day of his Accession, Lord Uacax followed the morning birds and then honored him with their song."

This leads to 12 hidden bird statues scattered around the Five Hills area. The first is just east of Samuk. You must visit all 12 in a specific order, hugging the walls near each statue to register the visit (no emotes needed). After completing the circuit of all 12, return to the first statue in Samuk. The Rusted Old Bell drops as your reward.

Jabbering Gemstone (Gibbering Gemstone)

Hint: "The Reverence of Uacax" found in Kurast Docks

The lore reads: "Although Lord Uacax was an orphan, he held great respect for his ancestors. Before embarking on his quest, he honored them thrice."

Head to the shrine just south of Kurast Docks (the one where you first speak with Akarat during the VoH campaign). There are several graves here. You need to use the Thanks emote at three specific graves in this order: middle, middle-left, middle-right. Emoting three times at the same grave won't work - it must be three separate graves. The Jabbering Gemstone drops after the third emote.

Crooked Staff (Staff of Herding)

Hint: "The Triumph of Lord Uacax" found in Kichuk (after clearing the stronghold)

The lore reads: "A cunning beast terrorized the plains. Though Lord Uacax was untrained in combat he hunted and defeated this foe."

Near Kichuk, there's a Super Elite Lacuni named Blood Lightning. You need to kill it using only the default basic attack - the one you have at Level 1 before spending any skill points. To do this:

1. Unallocate your entire skill tree
2. Remove any gear that grants Skill Ranks
3. Dismiss your Mercenary (they can prevent the drop)
4. Kill Blood Lightning with the default attack

The Crooked Staff drops on kill.

Corrupting the Relics

With all three relics in hand, head to the Field of Giants in Nahantu, just north of Ichor Falls. There are three Cordyceptic Zombies in this zone. You need to kill all three near each other - killing them alone won't work.

When killed near other Cordyceptic Zombies, they produce impassable Black Mushrooms. Drop your three relics from your inventory into the mushrooms. You'll see a purple and black aura form around them, confirming they've been corrupted.

Tips for this step: Clear the area around the zombies first. Start at the northernmost zombie and work clockwise, dragging them toward each other. They have a massive aggro range, but you need to keep them on screen or they'll reset. If you accidentally kill one while clearing, it respawns immediately. This step is much easier with a friend helping to kite.

The Unusual Key

Take the three corrupted relics to the Oka'bo Temple, southeast of Kurast Bazaar. Near the entrance are three braziers - interact with each one to offer a corrupted relic. All three will light up, and you'll receive the Unusual Key.

The Forlorn Burrow

Head to the Forlorn Burrow cellar in Nahantu. At first it looks like a normal cellar, but with the Unusual Key, a breakable wall appears inside. Smash through it and you'll find a hidden room full of cows.

Kill every cow in the hidden room. The final one drops the Rusted Bardiche - the Bovine Bardiche from Diablo 3. It's technically equippable as a weapon but has terrible stats and its tooltip notes it makes you reek of manure. Save it for the Horadric Cube recipe.

Step 3: Neyrelle's Hand (Lord of Hatred)

This is the newest chain, added with Lord of Hatred. Despite being the final piece of the puzzle, it's actually the most straightforward of the three. It revolves around burning four sins at the Kyovashad gatehouse shrine, each unlocking a different chapter.

Sin 1: Pride - Father's Chains

Burn Pride at the shrine. Find the Outcast's Journal on Mount Hifosseras. Defeat ballistas in Acarat's Bulwark to get an Amazonian Bolt, then enter the Idylic Reach cellar. Fire the ballista at the entrance, climb the rope, and defeat the Jailer of Angels. The Father's Chains drops. Burn it at the brazier at Antia's Flame.

Sin 2: Anger - Mother's Rose

Burn Anger at the shrine. Beat on a training dummy in Teganze until the Amazon Training Helm drops. Equip it and hunt corrupted Amazons in Acarat's Bulwark, equipping each armor piece (tunic, pants, gloves, boots) as it drops. In the full set, kill Captain Wassa for the Mother's Rose. Burn it at the brazier inside the Temple of Courage in Athulua.

Sin 3: Fear - Hierophant's Skull

Burn Fear at the shrine. Dismiss your mercenary and head to the Ruin of Broken Reason on the Isle of Lyhander. Do not attack any enemies in the area. Speak to the crow at the southwestern fountain, then chase it across the map without losing it. The crow eventually returns near its starting point and drops the Hierophant's Skull. Burn it at the Temple of Life brazier. A Horadric Key drops.

Sin 4: Greed - Neyrelle's Hand

Burn Greed at the shrine. Head to the Creaking Hulls shipwreck north of Hawezar. Stand on the lighter-colored wood at the end of the wreck and fish from that exact spot. A coffin appears - use the Horadric Key to open it and enter Rathma's Temple. At the altar where Elias's Mummified Finger was found during the campaign, search the nearby area until Neyrelle's Hand drops.

Step 4: The Horadric Cube Transmute

Take all three items - Stamina Potion, Rusted Bardiche, and Neyrelle's Hand - to the Horadric Cube in Teganze. The recipe will show three question marks. Transmute them to receive the Trophy of the Faithful.

Open it (it's a cache). Inside you'll find:

• Torn Page - your ticket to Cow Island
• Moo emote - unlocked permanently
• "There Is No Cow Level" achievement
• Moo player title

Step 5: Sailing to Cow Island

The Torn Page describes a hidden boat on the eastern shore of Skovos. Head to Zerae's Shore in the Philios area and look for a small wooden rowboat called the Outcast's Boat along the eastern edge of the coastline.

Board the boat and enjoy the cinematic voyage. You'll arrive at your own private Cow Island, complete with humanoid cow statues, a "Do not feed the animals" sign, and a cellar entrance.

Step 6: Opening the Cow Level Portal

Here's the catch - the actual Cow Level portal can only be opened on Tuesdays in your regional time zone. This is almost certainly a nod to the infamous "Damage on Tuesdays" affix meme that plagued Diablo 4's early itemization, where players would find affixes with absurdly specific and useless conditions.

On a Tuesday:

1. Return to the Kyovashad gatehouse shrine and select the Greed option again.
2. Sail back to Cow Island.
3. Enter the basement.
4. Slaughter the 12 cows arranged around the rune in the hecatomb chamber.
5. The blood explosion opens the golden portal to the Secret Cow Level.

Step inside and meet the Cow King.

How to Bring Friends

If you've completed the full chain and a friend hasn't, you can give them a shortcut:

1. Find your friend in-game.
2. Use your Moo emote on them.
3. Have them travel to the Kyovashad gatehouse shrine (the one from the campaign opening).
4. After being mooed at, a new Moo option appears on the shrine. They select it.
5. A short cutscene plays, then they head to the western mountainous area of Scosglen.
6. On a specific shelf they'll find the Outcast's Journal and a golden portal leading directly to the Cow Level.

Important: The shortcut only grants combat access. Your friends will not receive the Moo emote, the title, the achievement, or their own boat to Cow Island. They'll need to complete the full chain themselves for those rewards.

Rewards and Drops

Cow King's Crown (Mythic Helm)

The Cow King drops a mythic helm called the Cow King's Crown on every kill, even on Normal difficulty. Its affixes are intentionally absurd - conditions tied to days of the week, hooves, and your positioning relative to the equator. It's a collectible and a flex piece, not a serious gear upgrade.

The crown cannot be salvaged for sparks and cannot be masterworked normally, but it can be sanctified for Indestructible (the most popular option).

Other Drops

• Prime Rib "quest" item?
• Fresh Milk healing pools (replace health pools on the island)
• Standard endgame loot scaled to your difficulty (works on Torment 12)

The loot density is intentionally lighter than D2's classic Moo Moo Farm. This is about the journey and the meme, not the farming.

Farming Tips

• The Cow King spawns randomly in the level - clear toward the back where he's most likely to appear
• Leave and re-enter to reset the instance (some players report a short cooldown - start a fresh session if needed)
• The Crown drops every kill, so one run is all you need for the item itself

Quick Reference

After two years of the community hunting for this, the cow level is finally real. It's absurd, it's convoluted, it only works on Tuesdays, and it's perfect. Moo.
