How to Unlock the Howling Mouth of Madness Portal Cosmetic

Thanks to members of the Diablo 4 community such as JorDani and xGarbett, we now know how to unlock the Howling Mouth of Madness portal cosmetic effect, tied to the "Only the Mad Run Free" Feat of Strength. The in-game description reads: "Decipher the fallen Amazon's message and unlock her secret reward in a run of Echoing Hatred."

Here's exactly what you need to do.

Requirements

You need a Trace of Echoes to enter the Echoing Hatred game mode. These are a rare drop, so if you have one, make sure you're prepared before using it.

Shrine Activation Order

During your Echoing Hatred run, you need to activate specific shrines during specific tier windows. The order is:

Tiers 1-9 - Activate a Protection Shrine

Tiers 10-14 - Activate an Artillery Shrine

Tiers 15-19 - Activate a Lethal Shrine

Tiers 20-24 - Activate a Channeling Shrine

After activating all four shrines in the correct tier windows, a Treasure Goblin named Flotsam will spawn. Kill it to unlock the Howling Mouth of Madness portal cosmetic and complete the Feat of Strength.

Tips

• You need to survive to at least Tier 20 (Torment III) and find the right shrines in the right windows, so make sure your build is strong enough to push that far
• Scout out each shrine location ahead of time, so you know which one you're clicking
• Since Traces of Echoes are rare, consider planning your run ahead of time rather than going in blind
